Got an in lab sleep study in 2016 and results were 0 AHI, nothing unusual was seen. Slowly kept getting more and more tired. Ended up eating healthier by consuming less sugar. Was no on any meds, no drugs, no tobacco and no alcohol. Weight has stayed the same at 155 lbs and 5’11”. Got blood work done, Vitamin B12 and D levels, everything was good. As far as thyroid I have gotten the free TSH and it’s low and normal. I started eating even more healthier, paleo, no more bread and fewer carbs. Cut back on caffeine, no caffeine after morning and less than 150mg per day. Still no improvement. Have had nasal congestion due to swelling, allergy test came back negative. Got nasal surgery to open up nose and still no improvement in sleep. Had two at home sleep studies in the last year. Before nasal surgery 5.5 AHI, after 8.3 AHI. I even bought my own bilevel CPAP and used it for a month making adjustments via OSCAR and getting help online, no improvement in fatigue. I have a history of TMD, headaches, anxiety, bruxism, GERD. I’ve tried pretty much everything: more exercise, supplements, diet, reducing blue light, white noise. I just ordered a new mattress so maybe that it, idk. Either that or maybe I do have a thyroid issue or it’s stress. Only real symptoms I have now are chronic fatigue, irritability, and anxiety.

Every once in a while I do wake up with a headache, seems to be more when I sleep on my back. I try to sleep on my side. Sometimes I sleep the whole night without waking up and feel more refreshed but still tired. I’m using a weighted blanket, that seems to help some. It seems like I usually just struggle with trying to get comfortable. Like with my pillow or because laying on my side or getting too hot. AC is set at 70f at night and I sleep with a fan. Hopefully the new mattress will keep me cooler. I’ve been to several doctors and also sleep specialist and immunologist. I just seems like nothing really is ever found. For a while I was having RA symptoms too, joint pain in hands and I am 35.

          Welcome to the forum, @weiss27md!

Have you been diagnosed with chronic insomnia or any other sleep disorder?

          A few questions:

          1. What time do you go to bed at night?
          2. How long does it usually take you to fall asleep?
          3. How much time do you think you spend awake during the night AFTER first falling asleep?
          4. What time do you get out of bed to start your day in the morning?
          5. How many hours of sleep do you get on an average night?

On work nights I go to bed around 9:30 pm and wake up at 4:30 am.
On my days off I usually go to bed around 11:00pm and wake up around 7:00am..
When I lay down to go to sleep I usually fall asleep within 15 minutes.
I would say on average I get 7 hours of sleep a night.
I spend on average 15 minutes awake during the night. I wake up 0-3 times a night.

I have not been diagnosed with any sleep disorders. Just the 5.5 and 8.3 AHI on at home sleep studies shows very mild sleep apnea. The thing is bad sleep can show up as AHI on a sleep study.

It might be worth speaking with your doctor to see if taking another sleep study would be appropriate or if something else is going on.

I wouldn’t typically expect to see someone who is active during the day and averaging seven hours of sleep with good sleep efficiency to experience such intense feelings of fatigue. I am not a doctor and my area of expertise is insomnia. You don’t seem to obviously or strongly fit the diagnostic criteria for chronic insomnia.
