I had some health issues about 1 year ago that triggered a very difficult bout of anxiety and insomnia. During those initial restless nights, I started tuning into my breathing and developed a hyperawareness. The minute I close my eyes, my mind drifts to noticing my every breath. This has stayed with me almost every night for the past year. If I am very very tired, I can usually fall asleep but when I wake up in the middle of the night, I immediately tune back into my breathing pattern and it becomes very difficult to fall back to sleep.

Has anyone else dealt with a problem like this and are there any CBT tools to help alleviate this problem?

Have you tried getting out of bed when you wake in the middle of the night and find yourself struggling with sleep? Doing this repeatedly, in combination with a regular and appropriate sleep window, can help lower arousal and the accumulation of sleep drive can help reduce the amount of time you spend awake at night.
