For months, I’ve had this problem where a shock feeling in my head keeps me from getting to sleep. I think its anxiety and I’ve been diagnosed with PTSD. Later in the day, my startle reaction seems heightened – noises around the house trigger a heart pain. A couple of physicians have diagnosed it as PTSD. I can see now that about 18 months ago, my anxiety was worsening around sleep. I was taking .25mg of xanax some nights, and trazadone every night to try to get to sleep. Just 3 months ago, I was taking 45mg of remeron and 150 mg of trazadone, and some nights 50 mg of atarax, to sleep. And about once a month, I wouldn’t get any sleep. Got off all of that. But still have some nights each week of very little sleep. And, those brain zaps hit me each night. Some nights, I just can’t relax through them. I’m not on any antidepressants, so that’s not the source of the brain zaps. Anyone else have these?

I wonder if you might be experiencing hypnic jerks?

As you suggested, the heightened sensitivity to environmental sounds could be connected to heightened arousal. Are you being treated for PTSD with anything other than medication? You might want to ask your doctor whether they think you’d be a good candidate for cognitive behavioral therapy for insomnia (CBT-I).
