    Welcome to the forum. Hypnic jerks are not uncommon and might be associated with high levels of anxiety/arousal and/or self-monitoring for sleep. Did you experience them six years ago, when you had your initial difficulties with sleep?

    The good news is that, since you overcame insomnia before, there is no reason to think that you won’t be able to do so again — despite the hypnic jerks.

    How did you improve your sleep last time around? Did you implement CBT-I techniques? Sleep restriction, in particular, can be helpful for hypnic jerks since building up high levels of sleep drive can help overpower the arousal system and reduce their frequency and intensity.

    Since you are a client, I would recommend contacting me directly with any questions or concerns you have as you progress through the online course.

    If you are still feeling fatigued after seven hours of sleep at night, this could be a residual side-effect of medication and would be worth discussing with your prescribing physician.

    Napping during the day reduces sleep drive and can also prevent sleep drive from recovering to a sufficient level for sleep the following night — so, if you are not satisfied with your nighttime sleep it’s usually best to eliminate daytime naps (or, at the very least, limit them to around 20 minutes and take them as early in the day as possible).

    I would not suggest documenting how you are feeling every single day since this can lead to a preoccupation with sleep. When we spend too much time thinking about sleep and evaluating our sleep, we can actually make sleep more difficult.

    If you are experiencing heart palpitations or any heart-related discomfort, you should speak with your doctor. Typically, insomnia is only considered to be a chronic problem when you have trouble falling asleep, staying asleep, or getting restorative sleep for at least three months.

    It can definitely be helpful to remind yourself that sleep is a core biological process that can’t be controlled or suspended indefinitely! The body never loses its ability to sleep. However, sleep can be disrupted due to high levels of sleep-related worry and anxiety, and by any attempts to control sleep.

    So, changing the way you think about sleep (the ‘C’ in CBT-I) and changing unhelpful behaviors (the ‘B’ in CBT-I) can really help you take steps to improve your sleep for the long term.

    It’s great to hear that you are waiting until you are sleepy before you go to bed. Are you also making sure that you get out of bed by the same time every morning?

    Is your main challenge falling asleep at the start of the night, or waking during the night and finding it hard to fall back to sleep?
